This is cumulative across all runs. Scratching curtains serves no purpose other than to give you a Treat to spend somewhere. It's easy enough to ignore this mechanic in Goals mode and not really make much progress, but it's a good idea to do this often, especially early in Goals mode, to get more Treats so you can unlock more perks. If you don't finish this in Goals mode (you likely will), you will make heavy progress in Procedural Mode when you grind Pawliday Den. Often, one of your goals will be to scratch five curtains, so you'll easily make 50% of the progress just from natural grinding in this mode.
