As far as I can tell this achivement is story-related and cannot be missed.

True story! I definetely explore a lot and pace main + side missions + collectibles.
I'm also looking at TA timer and not in-game... Which may not be the smartest....

The, Pants, Party
You will get this quest as you exit the first town of Farnsport. Once you reach Tormund and find the fast travel crystal on the eastern side of the city, you'll be able to enter the guild just above it and complete the quest to unlock the achievement.
