You need to ged rid of all faded parts of the map.
If you think you have got rid of all faded parts but you did not pop the achievement you have forgot something that's for sure.
I checked all levels 3 times before I spotted the last place that I was missing.

You need to collect all 72 skulls to get to the hidden level on level 2-5 'the prismatic eye'
Pick entrance C (go to the top) when you have enter the exit you can get the last 3 skulls and get rid of all faded parts for that level also.

Everything in this game can be done solo except the honeymoon achievement hope this help you

It would be awesome if you could post images of all the completed maps. I've triple-checked and I don't see anywhere on any level that doesn't have a full border.

I can't really remember for sure what level it was but i remember it was a little faided part on the map that was very hard to see, I think it was chapter 2/5.
Take your time look because it's easy to miss...

The new update adds a map discovery percentage counter to each level, should make this achievement a lot less of a headache :)

On level 3-3 RIGHT to the right of entrance B is a faded area of the map that goes directly up. It's a spot for two players to get a blue gem. How on earth do you get up far enough to reveal this part of the map??????

Whilst it can be done solo, this is absolutely a co-op achievement, just spent over an hour trying to get last bit of 5-1 but the co-op puzzle at the start can't be solo, least not as intended.

What map was the last one you completed? Or, what are common maps that are likely to be missing a small piece?
I've spent a couple of hours looking over my maps and can't locate anything missing. :-(

There are a total of 25 maps (including the hidden map 5-5). In order to reveal all the maps, you will need all the entrances unlocked. Finding alternate exits grant you new entrances.

I made some video guides you could use, you can see every map fully revealed. You'll have to pause if you want to have a thorough look, as I had to go through them rather quickly.

(1-1) - (2-2)

(2-3) - (3-4)

(3-5) - (4-5)

(5-1) - (5-5)
