As long as you search every room you should find every crate (or crate like object I.e urns, boxes, barrels) in the game. And I believe that while they exist in the secret rooms they shouldn’t count (pls correct me if I’m wrong) so don’t worry about them

“Crates” actually mean anything that is breakable and does not respawn (so lanterns and torches that hang do not count). These can be boxes, urns, barrels, etc. When looking at your map, a small dot in a room means an unopened chest or a crate/breakable that you missed. Simply break everything to unlock the trophy. You have to break all of them on one save file.
