Here is a pretty detailed guide on how to obtain "THE GOOD ENDING"
Nothing but spoilers ahead.

I had 5/5 Team members gathered, 5/5 Tools acquired, 4/4 Investigations conducted, 4/4 equipment
All 5 Team member recruited is a must, 2 uniforms (A FEW THING FROM AN OLD HIDEOUT and CHECKPOINT ASSAULT) and explosives (HARDWARE FOR REAL MEN) is a must. You also need Semonov's secret passage map and Kozlov's password.
My companion relations were:
OLIVER- GOOD (this is a MUST for good ending)
MIKHAIL- NEUTRAL
SASHKO- NEUTRAL
TARAKAN- GOOD
OLGA- GOOD
You can play the game how you like, but before going on last heist build a RADIATION CHAMBER and change your memories (each memory change costs 3 chernobylite, farm in Monster killing missions if needed) to this:
RIGHT SIDE
A MEMENTO FROM THE PAST- Destroy Duga Radar (might change this in late game to use that upgrade until you recruit Tarakan and learn no cooldown skill for your PDA)
MEETING WITH TARAKAN- Agree with Tarakan and burn documents
THE CRUMB TRAIL- Ask him to join
A FEW THINGS FROM AN OLD HIDEOUT- Demolish the hideout
AN OLD FRIEND- Confirm
CHAIN REACTION- Decline surrender
SOME EXCUSES- Kill Him
TRAITOR- Stop him
A VOICE IN THE WOODS- (left) Lie to her face, then Ask her to join you, (middle black), (right black)
ON THE OTHER SIDE OF THE BARRICADE- Release Glyeb
THE TRUTH ABOUT NAR'S EXPERIMENTS- Destroy the data

HARD TRUTH- (left) Let him stay, (right) Tell Kozlov the truth
HEIST:
Oliver and Sashko- Infiltrators
Olga- Sniper
Tarakan- Hacker
Mikhail- Spy

Entrance- Pretend to be prisoner
Biometrics gate- Overload the gates
First locked door- Kill the sniper (after you open door, alarm sounds, it is fine)
Electrified water- Cut the power (long run through corridors)
Guards- Try to fool the guards
Scientists- Agree to the spy plan
Second locked door- Use alternate passage (crawl through vents)
Third locked door- Blast your way through
Alerted guards- Kozlov sends them away
Oliver reveals his motives- if your relations with him are good, he believes your promise, if not, he kills you
Black stalker- Boss fight, I used sniper build AK with red dot scope and max damage Shotgun, stay in corner and chip away at his health

The last boss fight. Don't forget to take a lot of weapons (there will be plenty of ammo on the stage, but not weapons). This achievement cannot be missed, the difficulty level and the choices made during the gameplay doesn't matter.

I seem to be stuck in a loop. Trying to start the heist with 5 team members I keep being pulled back to choosing members again - Base is covered in vines and no matter what choice I make in terms of assigning members. i can't actually start the heist. Any ideas please?

I died somewhere on this level without any form of combat because “I was not well prepared” and was send back to that other dimension to change some choices I made along the way.
I have to mention , because I did a mop up BEFORE the last mission I lost 2 members of my team because I needed different outcomes of the missions for some intel and clues. . I think I have to get the team complete to get to the end. So I would say difficulty does not matter , the choices made during the game do matter because it probably wants you to have a complete team of 6 including yourself. However , I have no idea if the questions in the beginning (who will do what kind of task during the final mission ) have any influence.
Nothing missable but now I have to figure out what choices to change to get my team complete again to go on to the last heist
