Unlock each level in 'Time Mode' by getting 50% coverage each level. Once you've unlocked all five, go to 'Free Mode' and start each level, place a single piece and then press start to pause the game and choose 'Exit Game'.
After placing a piece on the final level, the achievement will pop.

I've actually found that you don't even need to place a piece when completing the levels in Free Mode. I just kind of sat there, waited until the game started, then quit.

When you first start the game, you'll find that all levels are locked except the first on Timed Mode.
To unlock the next level on Timed Mode, you must get 50% coverage on the current level.
To unlock the same level in Free Mode, you must get 50% coverage on its countarpart in Timed Mode.
For example, get more than 50% coverage on level 1 in Timed Mode to unlock level 2 in Timed Mode and level 1 in Free Mode.
As the achievement says, simply unlock all levels and play a game on each to unlock this achievement.

Do you have to pass each level in Free Mode?

There isn't a way to 'pass' each level in free mode as there really isn't an end. Instead, start the level up, place a few shapes and then press start and exit back to the menu - this should be sufficient for the achievement.

In order to move on to the next level in Time Mode you'll have to cover at least 50% of the grid using quads. Once you've gotten 50% coverage on all 5 levels in Time Mode you can jump over to Free Mode and play each one to unlock the achievement. In Free Mode you simply need to load up the level and play for a few seconds, then back out.
