How to unlock the Baker's Dozen achievement in Chivalry 2 - Definitive Guide
New:
CPC mentions below that there is now a ‘bot brawl’ mode where you can easily grab bread and throw it at AI enemies.
Old:
This achievement requires you to kill someone by throwing bread at them... 13 times. You can’t kill them by punching them while holding bread. You have to throw it at them.
Here’s what I’ve been doing: find someone on the ground or beat someone with your primary weapon; throw bread at them. +7% on the tracker. An additional semantic; they must die. IE; no knockdowns. If they’re knocked down, you have to kill them w/ bread. If they’re standing, they must die on impact.
Siege of Rudhelm is by far the best map; with at least 4 bread spawns. One inside of the gate by the camps at the ‘burn camps’ objective, one on the push siege ramps objective nestled up by the walls in a small building, one right next to the courtyard during the capture objective, and one inside the castle.
The tournament map has one bread spawn on the side where the kings throne is.
Edit 2: Here’s a new one I found today!
Coxwell; right by the marketplace in the second to last phase of battle.
Edit 3:
Here’s a video showing a bread spawn right next to the town center that you need to capture/defend in Siege of Lionspire. To boot, I also show an example of killing someone with bread. This was recorded when I was going for it so I verified a percentage increase on the tracker right after recording it.

Here’s an additional bread spawn by the same objective. Circle over the bread spawn, big arrow pointing to where you can see the objective by the building.
This’ll probably be my last edit on this, just know that there are a ton of bread spawns and there are maybe 3-4 extra spawns not mentioned on this guide.
I personally couldn’t find any bread in the Fighting Pit, Battle, or Dark Forest. If this guide still isn’t helpful enough, please comment and I’ll add more.
Edit 4:
I lied, I’m back. There’s another two on Lionspire. Left side on the objective to take the bridge and destroy the banners right after ‘town square’. It’s in a little tower on a side table right at the start.
As MarkyShizzle said in the comments, during the final objective to destroy the trebuchets, there’s some bread on the large table inside.

A small clip of locations for all maps would make this guide perfect

I’ll keep adding more to it.

for this you wanna want to go on to free for all (Tournament grounds) and go to the right side of the map. on the right side there is a staircase that leads to the bread, simply pick up the bread and try and find all the bots in your game (best playing late at night or early morning so more bots are in your game) and hit the bot once and then throw the bread at them
Couple days ago in Chivalry 2 was added new Brawl mode - Fight Knight. Player is a guest during a party when everyone fighting with everyone. At the center always is a giant table where you can find a lot of bread. Kills in this mode counts (works for me).

Note: you must get your kills by throwing the bread
The easiest way to do this right now is to tab over to offline mode and select offline practice. I can confirm this is how I got all my kills but one. Load up Lionspire and fight your way into the castle. As soon as you get into the castle walls look on every table you see, bread is everywhere and this is easily farmable. Select your one handed weapon of choice and pick up your nearest loaf. Hit the bots until they get visably bloody and then toss that roll at the bloody bots 13 times for your achievement. You can use one piece of bread for multiple kills.

Mid Summer Tavern Brawl Has Infinitely respawning bread on the tables - Use it!
You Must Throw the Bread, It Must Kill the person, Not knock him down.
If that game mode is not available the map with "Defend the Hamlet" Objective has bread on the hamlet tables, and Lionspire has bread on various tables as well, but the chance of a player "Going down" instead of dying from the bread throw is higher in these maps.
Goodluck!
