How to unlock the Rise as One achievement in Chorus - Definitive Guide
Before I describe the final achievement I want to highlight a solution for any who are having difficulty with achievements not unlocking in this game. My achievements, both story and progress, stopped unlocking about halfway through the game. I tried deleting and reinstalling the game and my save file to no avail. When I logged on to play on the Cloud via my phone, and continued my latest save, the retroactive achievements would unlock. I would make progress on my console, and then save, quit, and reload the save on my phone to get the achievements to unlock.
This included the mastery achievements. Their progress would not move even after bringing a skill to level 5. But when loading a save with the appropriate mastery level in the cloud gaming system, the progress bar would then move on the achievement list, or the achievement would unlock if fully completed. I hope that can help others who have had the same issue.
This achievement is the last of the story achievements and is unlocked by defeating,
I found this level more difficult and annoying than any of the others before it. The final boss sequence is broken into five stages.
1. You fly through a long chamber with moving pillars trying to damage the final boss. I constantly used Rite of the Storm to weaken shields and then chip away at the health with my gatling gun. In hindsight, Rite of the Hunt may have been a better option.
2. The final boss becomes one with the Faceless and becomes a Faceless serpent. You have to destroy 4 blisters on 3 separate occasions. I found this part the worst of all because you are spammed with swarms, homing lasers, meteors that spawn directly in front of you. Additionally, the serpentine motion makes it difficult to hit the blisters. After destroying a total of 12 blisters the final boss sits in the middle of the stage and you have to use Rite of the Star to continue to damage.
3. This is a repeat of stage 1
4. This is a repeat of stage 2 but with the addition of energy fields.
5. You become supercharged and have to destroy 5 circle ships and then destroy the final boss once and for all.
This included the mastery achievements. Their progress would not move even after bringing a skill to level 5. Would have been a great game if it didn't have glitchy achievements and had a few more areas so you didn't have to grind out hundreds of kills in the last mission. I had all 3 mastery achievements and the last two story achievements glitch out on me. It took beating the last few missions and game 3 times on different old saves before I got them to unlock. Still can't seem to get mastery of combat. As the game is right now, I'd avoid it if you are an achievement hunter.

I agree with the OP about the unlocking however I found for all except the perma death I could just quit the game. If I came back to the game the next day, multiple achievements would pop and again I agree this is from around the middle of the game. It is a bit of mix of a game - I really enjoyed parts of the game but the voice acting/ whispering and lack of direction at times and yes the multi level bosses especially the last one - tedious. Maxing the cheevos is not hard if you are diligent working through the masteries during the normal gameplay it saves a lot of grinding later. Would I recommend it - well I certainly wont do another play through and thank goodness perma death game completion was not a cheevo!!!
