Playing with a downloaded copy from Game Pass on XBox Series X.

Before the final phase of the game I had already levelled up all my Rites through normal game play progression (as soon as I got a new Rite, I would use it as much as possible to avoid issues later, the same with the Combat mastery). I had also collected all the Rite fragments, some of these are available after completing the main story quests. There are also opportunities via the Faceless rupture phenomena, usually a pink cloud will appear and there is a time limit to close it by means of 5 nodes (just fly into them), if you do it within the time you are awarded a shard, if not faceless craft will appear through the "rupture".

Note: I have been having issues with unlocking all the achievements in this game, they are very slow to pop and usually require closing the game, restarting the XBox, then hitting continue once back in the game, however and in some cases (this achievement was one) waiting until the next day before it will unlock, so its a bit difficult to be certain of all the unlock criteria.

In summary for this achievement I believe you need to have all Rites maxed out and all shards collected, and then complete the game (once you get in the final battle you will understand why you don't get the achievement earlier despite having all Rites and Shards).
