This is fairly easy to unlock. When playing online, the three warring factions; Democratic Republic of Tarakia, Republic of Morskoj, or Kingdom of Sal Kar; choose the faction which is currently winning. Play with the country until the end of the current Neroimus War. If your country wins, you will unlock the achievement.

The nation you’re fighting for destroyed another country. I recommend you join Morskoj, since they win most wars. After capturing the enemies capital, all their territories belong to your country and you’ll get the medal.
