Achieve a rank of S in all the 43 missions. Every mission has secondary goals, which when completed grant you the rank of S in the mission. You can either finish the mission the first time and then replay it, or open a walkthrough in gamefaqs so you can find out the objectives. I played the game with the guide and got most of the S ranks in the first try.
