How to unlock the Prison Break achievement in Cities: Skylines - Definitive Guide
You'll need just under $2million for this achievement. Just pop down 15 prisons and then the achievement will unlock.

still didn't unlock for me, after patch. even replaced all 15.

Confirming that this is working for me post-patch.
I've deleted my Solution where I mentioned it was Unobtainable.

Wait until you have at least $2,000,000 to spend. Once you reach this amount, save your game, and turn off auto-save.
Go to an area where nothing is built, and construct a long road. Place the 15 prisons (or less, if you already have a prison/prisons in your city). Once the achievement unlocks, quit your game. (This way you don't end up having to waste the money.)
Don't forget to turn auto-save back on once you continue working on your city.

To unlock the Prison Break trophy in Cities: Skylines, you will need to play the game normally until you reach the level of Big Town, which unlocks at a population of 1,800 - 7,500 depending on the map you are playing on, so that you can unlock the Prison buildings.
You will then need to save up your money until you have 1,800,000 (prison cost 120,000). Once you have enough, simply place 15 prisons on the map and the trophy will unlock.
