You gain temporary glitch levels that cause a chance to roll glitched dice by repairing broken dice (a feature that becomes available very soon after you escaped Hexport). Sometimes you also gain temporary glitch levels in scripted events in which you are damaged.
On standard difficulty, you earn permanent glitch levels as a penalty for dying (which you should avoid and which isn't necessary for the achievement).
A glitched die has a chance of 80% for a negative and 20% for a positive outcome, regardless of proficiency, so using them on a critical check is usually a bad idea. Just keep using them on safe activities or on repeatable hub activities that only cause energy loss on a negative outcome until you get lucky and score a positive outcome. This isn't really missable unless you avoid rolling the glitched dice on purpose, you'll likely get a lot of dice repairs and thus glitched dice during your playthrough.
