Toward the end of chapter 3, after entering Adas apartment, she will ask you to get her research by completing a puzzle. Start by looking at the paper on the table in the middle of the room, then interact with the nearby painting, followed by looking at the symbols on the polyhedrals spread around the room, and finally matching up the symbols and shapes on the lock. Enter the secret room and interact with the notebook for the achievement.

Chapter 3, after you reach Ada's room and decipher the painting code, go through the shaft, and on the right there will be the notebook.

When you reach Ada's room, pick up the photo on the desk and document on her bed to progress the story. Once dialogue with Ada stops you can pick up the document on the small table near centre of her room. Then head over and click on the painting to reveal the puzzle. There are 4 different shaped objects sitting around the room and each one has a unique symbol on it, use these to unlock the code and this opens an air vent just below the painting. Climb through and the notebook in just on your right as you get to the other side.
