Cocoon has 11 total collectibles known as Moon Ancestors. These can be acquired as you play through the game, or easily mopped up using chapter select once you finish the story. They are most often hidden just off the main path; may involve a small and simple puzzle, and are unlocked with an interaction.

Just after the start of the 14% checkpoint, go up the lift, and stay to the left to discover a hidden path with the collectible easily grabbed here.

CHAPTER 1 - OFF THE BEATEN PATH ACHIEVEMENT

You unlock this by interacting with the first Moon Ancestor in the game, you will find this in Chapter 1, this guide will take you through the first chapter, the achievement drops just around the 10:00 mark.
