In this video guide I will show you how to view Io's memory echos. I ended getting Io's memories shortly after beating the Successor of the Ribcage. You should automatically get this from beating the boss. Once all the cut scenes end, head back to home base. Talk to Io, this should start a little cut scene. Once you are asked to view her memory, do so. Once you finish the memory you will get the achievement.
Puppys
You will be able to access Io's memories after defeating the Successor of the Ribcage in the Cathedral of Sacred Blood. Talk to Io at home base and choose to restore Vestige, then choose any Eos Vestige. Remember to take into account that restoring every single one of her Vestiges in particular can affect the ending of the game.
