no idea why there are guides for the other endings and not this one, but here's what you need to do for the neutral To Eternity ending!
REQUIREMENTS:
For "To Eternity" you must restore the memories of 1 to 3 successor bosses before completing the story. If you restore 0 or 4, you will get a different ending and void this achievement.
DETAILS:
There are a total of 4 special bosses, each with vestige items that need to be collected. Here is a list of
area - boss- vestige.
Cathedral of the Sacred Blood- Successor of the Ribcage - Isis
Ridge of Frozen Souls - Successor of the Breath - Fionn
City of Falling Flame - Successor of the Claw - Scathatch
Crown of Sand - Successor of the Throat - Harmonia
RESTORE AT LEAST 1 AND AT MOST 3 SUCCESSOR'S MEMORY (NOT ALL 4):
Step 1: Each successor boss has vestige fragments scattered throughout their area. After all fragments are found, visit Io or an Io clone (an Io clone stands at the entrance of every successor boss room) and have them restore the vestige.
*NOTE* if you followed the TA walkthrough and got the good ending already, you've already collected and restored the vestiges.
Step 2: Defeat the boss
Step 3: At the end of the memory you are taken to, you will be standing in front of a statue of the boss character's human form. Walk up to it and press
.
Congrats! You've just restored the successor's memory!
Remember: for this ending you need to do this at least 1 time.
DO NOT RESTORE AT LEAST 1 AND AT MOST 3 SUCCESSOR'S MEMORY (NOT ALL 4)
Collecting and restoring the boss' vestiges are not necessary.
Step 1: Defeat the boss
Step 2: At the end of the memory, DO NOT press by the statue. Instead, exit the memory by walking towards the door in the background.
Congrats! You did not restore the successor's memory!
Again, for this ending you need to do this at least 1 time.
If you've followed these directions, you'll get the To Eternity ending and this achievement.
THE OTHER ENDINGS
Heirs (bad ending): do not restore any successor memory
Dweller in the Dark (good ending): restore all successor memories
Dweller in the Dark+ (alternate good ending needed for Determiner of Fate): restore all successor's memories + restore Eos vestige

There's 4 endings total and they're dependent on how many successor memories you restore:
bad: none
ok end: some
good end: all
good+ end: all + Eos vestiges.
this achievement and the solution I wrote is for the 'ok' ending. I included the requirements for the other endings just to be informative. I suppose I'll rewrite it a little bit to avoid confusion.

I don't think your "good end" is correct ... and your more complete explanation states this: you do not need to restore ALL successor memories, you need between 1-3.
I just completed this achievement and I restored two successor memories (not ALL). Otherwise, good summary.
