From the wiki:
"To start a looped run you have to reach the end of Level 1-1B. Here you will have to take down your Partner (e.g. using Dynamite) or tell them to guard a position (left on d-pad). Then you have to quickly move out of the level the way you would usually complete a run. Instead of a game complete screen you will now see that the game started to generate a new level. A looped run has started."
Starting a fresh looped run from the gear icon (which unlocks after you finish a run once), top left after selecting new run, will not unlock this achievement. You must loop after reaching 1-1B.

Just wanted to highlight that.I'd classify this as highly missable. It's all too easy to just walk to the edge transition, as you've done a dozen times, since there's nothing to alert you to the fact that you're actually crossing the finish line.
