Defeat the mutated HeHe at the end of volume 12 and the achievement will unlock after a brief cinematic.
The battle is similar to the first time you fought it, but with a few differences. It has a couple new attacks: One attack is where its chest rips open and its heart appears. As you shoot it, it will unleash a shockwave-type attack. Stand in one of the openings or you'll take damage. The second attack is where it grabs onto the trees on each side of the screen. Slide to avoid it.
Also, make sure you avoid the bubbles that the Kid Cupids spawn which move around the perimeter of the screen.
When its HP gets down to around 64k, CoCo will heal it back to full health. After draining its HP to 64k again, NaNa will heal it. When you get its HP down to 10k, LoLo will appear and distract the other two while you finish off HeHe.
