To impale an enemy, you have to grab him and throw him onto something sharp. The best and earliest place to do this on the second level: there are numerous spikes placed around the map, making this achievement a cinch to get.

There is something I'm missing here. I have tried this on easy, medium, and hard. I have picked up everyone on the pirate level and thrown them at everything that looks vaguely sharp. I hit "B", face a spike, and let him throw them. I have hit "B", faced a spike, and pulled "RT" and thrown them. I still don't have even a single impale!
In the Bone Cleaver arena, there seems to be an invisible wall that keeps the pirates from hitting the pointy parts of the wall on the right side. And I don't see anything sharp on the left side at all.

Sounds a little glitched :S mine popped on the pirate level (level 3?), while throwing an enemy at a wooden spiked fence, a little after a quick-time event opening a door (hitting B, then push forward on left stick, then hit B some more). After this point I threw pretty much every enemy I came across and after the stone path is a wooden path, with spikes on the edge. Popped when I threw an enemy at that bit. Maybe they have to die when they hit it? Mine was at standard difficulty, first playthrough.
