Unlocking the achievement is straightforward and is non-missable as it's a story-based achievement: just work your way up the levels cleansing the siphons along the way. After clearing the ground-level ones, if you die those will stay cleansed.

I had an even worse experience. the game crashed on me and the achievement never popped but the game has recognized me beating it already. I am gonna try mission select and hopefully that will fix this major glitch

Ugh. Yeah, I wish that replaying a mission would just use the state you're at now, rather than throwing you back to the state you were at when you first started the mission. Good luck!
