To get this achievement you have to get both the gems (break all boxes and complete the death route) in the new level Future Tense. To get to the Future Tense level you go up the new platform in the middle of the main warp area (located on the side not the direct middle platform, I know, kinda confusing.)
Additionally, it should be noted that to get the gems you will need to have gotten the bazooka due to a lot of boxes and "!" boxes in inaccessible locations.

This achievement requires you to obtain both clear gems in the extra level Future Tense. It is accessed by riding the little silver platform in the middle of the hub, off to the side between the first and second warp rooms. While this level can be accessed at any time, I highly recommend waiting until at least finishing the fourth warp zone, so you have the double jump, spin upgrade, and bazooka. They will make traversing this level and breaking all the boxes much easier.
Of the two gems you need to get, one is for breaking all boxes like usual, but the other is found at the end of a death route. This is a pretty tough level, and it requires covering quite a bit without dying, both to get to the death route and then to survive it and reach the next checkpoint so you don't have to do it over again. Additionally, this level is one of the only ones with some really sneakily hidden boxes. Definitely consult the following video, as you will otherwise assuredly miss some boxes even after trying to be thorough. Full credit to the creator of the video.
