Will unlock when you'll have the 20 hidden idols, all costumes and gold rating in every episode.
Difficulty does not matter BUT everything must be done on the same difficulty.
If the achievement do not pop, go to "Crash stats" and it should unlock.

One of the costumes comes along with the Gold Idol on every level, so basically the solution should be just be "get all the Hidden Idols and costumes on one difficulty," but detail is better.

no his solution is good, you have to open the crash stats to get it. Yeah.

Once your sure you have all hidden idols, and creature costumes unlocked, and gold on all 20 episodes just go to the Crash Stats and have a look , this will unlock once you enter the stats.
