Below you will find a list of all Entries in the Avapedia. This achievement is obviously very missable and will take a very detailed and organized run to get.
For the creatures you need to take a picture, pet the creature, take an infected picture, use co-op abilities, Cure infection, and rescue.

Something that doesn't make sense to me is that even before the last Têhra'ba fight, my save file says that I completed 100% of the Avapedia. I got every collectible in the game, took the photo of every NPC and got 100% on every creature, but I didn't get the achievement.

After my second run for 100%, I don’t have achievement.. but I noticed that I had 100% on the Tehra'ba but by clicking on it I didn't have the healthy picture, just pet and cure infection.. I guess it blocked my achievement

Yokai Rajin - Yeah same here I checked just now and seem you right this might issue. I hope someone here to tell developer about it and fix it for upcoming patch.

So I’ve rescued all Ohol on grasslands but only used ability twice - have I basically now made this achievement impossible to get on this play through?

I have collected all creatures in the grasslands, but seem to be missing one Kraj (15/16). The map seems to not indicate any other Kraj... has this happened to others?

If you miss the healthy Tehra'ba photo entering the Jungle region are you locked out of this? I'm in the dunes and have everything else at 100% just wanted to see if I should reset now, or if during/after the boss fight with Tehra'ba I'll be able to snap a healthy photo.

It's messed up of the devs to intentionally make it so that you must replay the game almost in its entirety, especially if you didn't get a pic of it infected which counts towards the trinket achievement, to take the one pic for this achievement I'm sure many gamers didn't even realize they locked themselves out of (just like with the wind chimes in the grasslands) since the devs thought it was a great idea to lock you into a cut scene after defeating the tehrabra at the end of the game.
This achievement only makes me want to rate this game a 6.5/10, as far as achievement hunting goes

The missable achievements in this game are an absolute abomination. I expected better considering the devs were interviewed by TA and should definitely know better.

I’ve completed the game twice making sure to collect everything in all biomes and completing all entries in the Avapedia and this achievement will still not unlock. Am I missing something or is the achievement bugged?

Is this legit bugged? I’m missing this one and the one for getting all of them because of this and my pedia is at 100%. (Revised. I loaded a save prior to the last fight and took a picture of it infected just for giggles and the achievement popped for me after the pet during the cutscene.)

All my Rescue Robots disappeared after a certain point near the end of the Swamp so I can't complete the Avapedia now 😮😮😮
I'm gonna keep a save here waiting for a patch/fix but continue on with the story as I'm really enjoying the game and am on course to get ALL OTHER CHEEVOS except the Avapedia one, the Trinket one and the All Achievements one 😔😔 I can always drop back to that save if a fix comes along...

Take care with "THE TREE IN THE AIR" entry in the "Others" section of the Avapedia. It is still bugged and after finishing the game I realized was the only one missing and it's supposed to be an automatic entry. Should unlock with the first mosaic (like 5 minutes into the game) and you are forced to restart the playthrough because of it. Let's hope the devs give some kind of support to the game and fix it so it unlocks retroactively or maybe you can do a New Game+ with all your previous entries.

(The location of healthy Tehra'ba)When entering the jungle, follow the path leading to the temple. Upon hearing the roar, look up and you'll see it walking on the cliff where wind chimes are hanging, lasting for about a minute.

Just a few tips for help with completing this achievement. I cannot stress, ensure all avapedia entries for each region (Jungle, Swamp,Dune) are completed before you complete it. The game indicates the point of no return for each region. I would suggest completing all of the grassland entries before you complete the swamp region. Reason being that when you complete the swamp region,
The main characters realize the bioark is being used for evil and decide to stop capturing creatures and if you haven’t captured all the grassland creatures it can lock you out of 100% the avapedia
Also make multiple stages before a specific region progression having a back up saved me from a second play-through.

It doesn’t matter how careful you are by completing everything before leaving the area if you don’t know how tricky is finding a healthy Tehra'ba.
This should be explain in the guide as first thing as disclaimer, since it’s the most missable photo of the game, that block you out of 3 final achievements.
I thought I could get it after having treated her since it’s always sick when you meet her.

“You can take a healthy photo of the Tehra-ba when you first enter the Jungle region. When you first enter you will walk through an open area outside towards some ruins to enter. The creature will walk across a large rock jutting out over the ruins. You pet the Tehra-ba after the final boss battle in the Mausoleum so don't worry if the avapedia card shows 66% for the majority of the game.”

Like @KyonDono, I got to then end of the game and didn't get this only to find The Tree in the Air hadn't been marked. I started a new game and discovered this should have auto unlocked when you complete the first Nafitar podium and so shouldn't have been missable. While missable achievements have annoyed people in this game its nothing to the annoyance of completing a game whilst trying incredibly hard not to miss anything only to miss something 'unmissable'. Be careful as you go with this one

this game is so buggy on my first playthrough I couldn't get it cause robots didn't spawn so I couldn't extract the amount of animals needed

I noticed that the only thing missing from the Avapedia is part of the entry for the Têhra'ba despite it being at 100%. The "healthy" image just has a question mark, because I never saw a healthy version. I just read that when you first enter the jungle you apparently do have an opportunity to get this photo. Time for 100% playthrough #3 🤕

Mongooseeyy - You can get a healthy photo later on in the game, when you encounter him again. In the jungle, he is infected.
Burke By Nature - I had to do the same. What I have found is that some creatures, like the Ohol and the Kraj, they share similar abilities which can make things difficult. I know there is an area where you can get 3 co-op abilities for the Kraj alone so my recommendation would be to run around and try to get the Ohol ones first. I had to restart because of this so I feel the pain.

No, Mongooseeyy is right. The first time you enter the jungle, you’ll hear the Tehra’ba roar right before getting into that first elevator. Look up above the building and you should see it. Snap picture, healthy task done. That’s how I did it.
You’re thinking about the one that’s infected towards the end of the jungle.

Should be worth noting that if you save/quit and reload you can reuse creature abilities on already used areas. Example fly an Ohol to an already lowered bridge and claw it again, should redo the lowering animation and add 1 to the counter. Can be done with any creature.

Can confirm: 3 playthroughs later, finally got it by getting the healthy photo of Têhra'ba at the entrance of the jungle - alongside everything else. This is the only opportunity as far as I'm aware 🤕

Currently on my second playthrough, just before entering the dunes. I'm missing only one location from the grasslands the "Tabitha's control center". Anyone knows where to get this ?
Edit : Figured out myself you have to start the Vaeh'ti quest to unlock this place (even if you've already be there multiple times)

There is no mention of the doctor's notes found at the old bases and other places around the map, but they are listed in the avapedia. Do they not count towards the achievement?
