Start a new Baker's Battle - the campaign. During the shootout at the beginning on the rooftop, you and your crew will face gradually increasing police forces. Allow yourself to be downed repeatedly. Eventually, this will trigger a fail-state and Norris and his robotic dialogue will chime in. Achievement. Come to think of it, I'm not sure if this death during the intro can even be avoided.

I got this while going for the restart the campaign 10 times achievement. Just start the campaign, then hit pause and scroll down to restart campaign. You should get an F for doing absolutely nothing and the achievement will unlock.
