This is achievement requires you to collect 33 Soul Anchors scattered among the campaign levels. There are 3 in each level except for the 2 boss levels (Level 5 and 13).
All of the Soul Anchors can be obtained while playing through a solo campaign. However there is one that is much easier with a second player.
The location of all 33 soul anchors are shown in the video below. In order to keep the video from being 4 hours long I've cut it down to roughly 10 second clips for each item. Unfortunately there is not a dungeon map like in Torchlight or Diablo so you will have to look for recognizable areas in the level.
Each soul anchor is listed in the order that they are found in without backtracking.

These are scattered throughout each stage, found in Secret Areas. Here is a video with all of their locations (I'm playing The Wizard who shows-up from time to time).
