After confronting the Time Empress in the ruins of Crystallis, you will need to revisit all the previous areas to obtain a key and deactivate terminals in the ruins you've seen on the world map. There are four total, and can be done in any order. Deactivating the last one unlocks the achievement. I was about 25 and a half hours into the game when I unlocked it.
