After obtaining the initial set of key chains through story progression, you only need one more. The last one is obtained by completing Dreena's parade quest in Neva Tulira. See that achievement for specifics. Alternatively, you can buy one in Cinder Town for 10k marbles.

There are four types of keychains in the game. There are a number of ways to get them, but you will receive three as part of the main story, and a fourth from Dreena's parade side-quest. You can't miss this while going for the other achievements and following the walkthrough.

There are few keychains in the game, you will receive all but one during the story, and you will receive the last one after completing the Drina side quest in Talir.
Or you can buy it at Cinder's store for 10k.
