Familiars are Elite enemies you've defeated which can be summoned to fight by your side. Elite enemies are identified by their large boss-type health bars inside the dungeon. There is one Elite per floor to defeat and you won't always fight a new one, so this will take multiple runs to get each Familiar in every zone.
This unlocked for me when I had 18 Elite entries in the encyclopedia.

How is it possible to unlock when you have 18 familiar entries, when you need 20 familiars unlocked?
