How to unlock the Bloody Line achievement in Crusader Kings II - Definitive Guide
You will need the Holy Fury dlc to reform a pagan religion. One of the options will be Bloodthirsty Gods which allows you to sacrifice any prisoner not of your same religion. You will then need to sacrifice a LOT of prisoners (I had almost a hundred and it took most of them). There are three tiers of trait (Haemophiliac, Haemophant, Haemoarch) each of which requires ~20 sacrifices to unlock and then an additional 20 or so to forge the bloodline.
I would recommend either starting Germanic (raiding gives lots of prisoners and it's pretty easy to reform at the early start dates) or playing nomads (requires Horse Lords; Mongols in 1220 or Khazaria in 769 are good options) to easily reform and obtain prisoners.
