How to unlock the Royal Flush achievement in Crusader Kings III - Definitive Guide
Disclaimer: I do not take full credit for this solution. This information comes from the achievements page on the paradox wiki. I only wrote the solution.
So it seems this achievement is somewhat bugged. According to the achievements tab on the paradox wiki, for whatever reason you can lock yourself out of the achievement on your current playthrough by either viewing the in-game achievements page to confirm that the achievement is possible during your run or by immediately advancing time when you first load in to a new game without first employing a master assassin court position.
Now, how to get this achievement.
Select the 867 start date and choose Sheikh Esmail of Bam. He's an independent single county ruler surrounded by the Saffarid Kingdom. Hit start and load in.
Once the game has loaded click/dismiss the Iranian Intermezzo popup but DO NOT advance time or check the in-game achievements tab. First, you want to go to your courtiers tab and find the Master Assassin position and hire the best candidate. Now you're in the clear and the achievement should not bug out.
To make the process easier swear fealty to the Amir-e of the Saffarid Kingdom, make sure that you have a religious exemption contract. Now all you have to do is succeed in a murder scheme against a lowborn, a king and an emperor. Find a random lowborn within your court and then look around the region for a king and emperor to target. Since it's so early in the game you will have a 95% chance against most kings and emperors within diplo range. Any random king within the middle east will do and for an emperor go with the Byzantine emperor.
Once you've succeeded in all 3 murder schemes the achievement will pop.

Thanks for the info. I did not experience that bug you describe during my playthrough as the Ghurids (who start as Bhuddists and whom I had convert to Azariquism later). Don't know whether the bug is fixed or whether it only occurs with rulers who already have the Fadaeyeen doctrine from the start.
I would recommend to do this achievement on the side during a longer campaign (e.g. one in which you aim to get the achievements for doing one of the endings of the Iranian Intermezzo), rather than starting a seperate game as a count for this, because succesful murder schemes against Kings and Emperors usually require a substantial amount of gold to bribe courtiers into becoming agents. Azariquism is quite a handy faith for campaigns in which you oppose the Abassid Caliph, because it has a high conversion speed (fundamentalism) and allows you to wage holy wars against both Shia and Sunni faiths.
By the way, the gender of the victims doesn't matter, a ruling Queen or Empress (title holder, not mere spouse) does count for the achievement.
