How to unlock the Trapped in the Web achievement in Crusader Kings III - Definitive Guide
Strong hooks tend to sometimes happen automatically but it is a
almost impossible for 3 of them to happen out of the blue. Luckily, there is a way to do this efficiently.
You need a character with Intrigue focus, and you need to unlock the "Fabricate Hook" perk in the Schemer tree. You will simply use this on your vassals.
Then, you need 3 vassals, but I suggest having a lot more as the Scheme can fail and you will then need to try it on other vassals (let's say 10 vassals should be enough).
Simply go and Fabricate Hooks on your vassals. If the Scheme is successful, there is a good chance that a Weak Hook is created. If this is the case, you have the option to continue the Scheme, it will then reset. Repeat until a Strong Hook is created. Immediately do it again for other two vassals as the hooks only exist for a few years.
I also advise trying this with Alfonso Jimena, the King of Leon in the 1066 start date. He normally starts with one Strong Hook on one of his vassals, and should have Intrigue focus.

The tip to use fabricate hook is good, but in my experience, you can get it done during a normal longer playthrough even without focussing on intrigue. Once you rule a mighty empire with plenty of vassals, there will be no shortage of vassals with dirty secrets that you can use for blackmail to get hooks. All you need is a good spymaster whom you order to find secrets in your court and a decent intrigue skill (or a spouse who can boost it for you) to make the blackmail attempts succeed.
Some secrets give you only weak hooks, but some do give you strong hooks, among them are murder and attempted murder (if the character isn't an independant ruler), disputed heritage, and lover for a married female character (only a weak hook for males - varies depending on whether adultery is considered a crime or is only shunned by your religion).
Also keep in mind that you can grant titles to characters on whom you already have a strong hook. Like e.g. if you have the medicine lifestyle, there is a good chance that you get a random event in which you can save a wounded soldier's life with a succesful learning check, who then joins your court and you get a strong hook on him.
