Select story mode chapter 1. After beating the first fight against Trevor, you will be allowed to choose Mr. Mayhem, also in chapter 1. As soon as this fight starts you will be playing as Ryan Johnson and will earn this achievement.

move over and over as I quickly found out I am rubbish at this game.
