How to unlock the Keen Observer achievement in Crysis - Definitive Guide
To use the binoculars, you hit Up on the D-pad. To tag an enemy, you must hold the box in the middle of the screen over an enemy for a little bit.
When you first place the box over an enemy, the enemy will be highlighted in white. If you continue to hold it over them (about 3 seconds), that highlight will change to green. At this point, the enemy is considered to be tagged. The highlighting will persist even if you move the binoculars off of them and they will be marked on the map and minimap. Note that if you get too close to an enemy, they will automatically be spotted by proximity. They can still be spotted using the binoculars as that proximity spotting will be lost when they get a certain distance away.
If you are spotting EVERY enemy before you are killing them, you should have this during the first mission, unless you are playing on easy or normal. If you are spotted, enemies that fire at you on those difficulties are automatically tagged, potentially lowering the total number of enemies you can tag.

I does auto-highlight guys shooting at you (on Normal anyway) but I don't think they count as I didn't pop this one until about halfway through "Recovery."

Great guide. Didn't know about keeping it on them for about 3 seconds to turn green.

You will be able to use the binoculars fairly early in the game after you've been told to do so by Prophet. From that point on simply press whenever you see an enemy and hold the crosshair over him to mark.

On the second level where Prophet tells you to use the binoculars just slowly progress yourself through the level until you tag enough soldiers for the trophy.
Press and go into binoculars mode. Then just aim at the enemy until he flashes white, then wait until he finally turns red so that he will finally be tagged. Just make sure you're not too far away otherwise it won't work.
Simply press up on the d-pad to bring up your visor. You can zoom in with L1 and tag enemies by just getting the visor reticule on the enemy (and it will auto tag them on your radar). Do this 30 times across the course of the campaign or chapter select. The count is cumulative.
