First you must have Sin and have two followers that are compatible (go for matches that are around 60 or higher because if its lower they won't have sex and you just wasted Sin) use the mating tent you then can choose which attributes that the baby will have (produces sin faster, poops become golden, etc.) then you must tend to the egg once per day (probably build the nest II for faster hatching) once you see a crack on the egg examine it and you now a baby. Do this five times and the achievement will pop

Just a heads up, when you're gathering Sin, if you gather too much from one follower at once, they'll call you greedy, tell you they'll haunt you from the grave, be devoured by Sin and permanently die.

Found that out as well Jack, my only advice for that is to not use the ritual to put all the Sin into a single follower once you have over 15 or so followers, maybe even less.
