One of the modifiers in Director Mode is an Inspector, a low skill (basically useless) character who will permanently take up one of your party slots for the rest of the playthrough.
They cannot be dismissed from the party and most things that would kill a party member such as disease or infection they will miraculously survive.
The most reliable way to engineer this is get your Sanity to 0 and then save. Trigger an insanity event and try to get the cannibalism event where one party member eats another. If you don't get this event, or the party member that is eaten is NOT the inspector, quit to dashboard before finishing the event and reload.
Once your inspector is successfully eaten they will disappear from the party, but will return out of nowhere when you finish the expedition, which will unlock the achievement.
Please note cannibalism can be a run ender on higher difficulties so you may want to do this separately from the A Paris Sunset achievement:
