This achievement requires you to upgrade all 4 upgrade paths for the weapon altars (Number of Altars, Main Weapon, Secondary Weapon and Two Handed Weapon).
This will require several thousand crystal skulls (I think around 4-5k). Initially, skulls come in very slow.
The best way to obtain skulls is to do the daily challenge runs, and to play through on the hard mode - doing so will frequently net you between 150-200 skulls a run. If you intend on getting all other achievements in this game, you will end your quest with way more skulls than is required to unlock everything.
