This will likely pop will fighting a boss, especially the Blitmate at the end of Guild Rank 1. Make sure your party members are alive and spam attacks, ignoring the boss' guard. Keep at it, dodging the boss' slam move (which will stop all of your party members) and use long range magic if you notice the combo meter isn't climbing any more and you'll easily unlock this trophy.

Your combo increases as you or your teammates hit the enemy. If you spend too long between hits, the combo will break. This will likely occur naturally against big bosses that don't move much. Definitely don't try this against flying bosses.
