Im not certain if this is specifically how this works but I have seen people posting issues about the achievement not unlocking for them.
For me personally - I killed an enemy as his grenade was still physically flying through the air towards me after being thrown, before it landed. This is how it unlocked for me - might be worth trying for those who are stuck.
This was very early on so I can’t say whether or not it needs to be part of a certain kind of job or encounter etc. Good luck

Having seen a play through video of this achievement, it does look like you have to kill them whilst the grenade is still in the air. This makes things a bit trickier but knowing what to do always helps!

Yeah, you have to be really quick and kill the guy right after he's thrown the grenade. Best done on easy difficulty. as soon as you see the grenade indicator, run up and shoot them in the face. I did it during one of the
sections.

You need a bit of lucky to get this achievement but I am sure you will unlock it playing normally in any mission.
This is for anyone who is having trouble getting this one to pop. After a 70+ hour play through I couldn’t get this to unlock to save my life. Tried killing an enemy as they threw it, after they threw it, after the grenade damaged me etc. and nothing worked.
Unfortunately you need to make a new save with a new character. Just make a street kid - the intro is about 10 mins skipping all dialogue.
After you meet Jackie, you’ll do the first mission with him. Very shortly into the mission you’ll come to an enemy you need to stealth kill. Do that and the next room will be full of enemies. Shoot one and just sit behind cover until someone lobs a grenade. Run up, kill them and the achievement should unlock!

A lot of enemies will throw grenades at you along the game, chances are you will get this trophy without even trying as it doesn't seem that you have a time limit to kill the enemies, simply need to kill them.
