If you don't get 5 stars during the campaign you can try again after you finish the campaign. Just find "Macarena" under Flash4wrd's songs in story mode.

I don't remember how many eras the gain contains, but I had to finish the 70's and 80's era before reaching this era. So you'll need to finish at least 10 songs before the Macarena.
