The requirements for this are easy: beat all 40 songs. You can do this in Career or Free play, and I'm assuming multiplayer although I wouldn't know.
But, this achievement is not story based. You actually have to get at least an Amateur rating in each song in order to get this achievement.
When I finished the career, there was one song I botched big time early on when I first started playing this game ("Bad Romance", which is the second song in the Career). I didn't get an Amateur rating in it and therefore I had to replay the song in Free play.
Note: You can check your scores in this game - it won't tell you how you ranked, but you can see how many points you got (your top 3 scores actually). "Scores" is one of the options in the menu. I knew that I had flubbed "Bad Romance", because I had a horrible score in it.

You may finish them in any mode and in any order. You will play all songs simply by completing career mode. You can also do this in Free mode as you can choose up to eight songs in a row to play.
You can check to which songs you've done by looking at the scores menu from the main menu.
