I thought this would be more complicated, but all I did was change the skin of each equipped item once. Just have a custom skin on each type of equipment active and it should pop right away. Will only cost 300 gold total at the merchant.

Right thanks. After the first mission I think upgraded to a helmet type that couldn't be reskinned, but I guess I'm 5/6 of the way there. Wish some of these achievements would record progress! Cheers.

Yeah, certain types of gear cannot be reskinned. It may be the elemental stuff?

As you progress through the game you will gain items. You can't re-skin your items you start the game with or elemental gear. Simply find an item for each slot (Pants, armor, cape, helmet, gloves, and weapon) and make sure none of them are elemental. If you need to, just save some gear you know you won't use but you know isn't elemental. Equip each of the 6 items and go to the guy who sells stuff. I believe if you press 
once it will take you to where you can re-skin. Then go to each individual item and for 50 gold each you can change the items skin. Upon doing this to all 6 your achievement will pop.
