I didn't have this by the time I completed the game, so I re-ran The Order of the One Light Act 1 (An Infernal Dispute) a single time - mainlining the level and killing all the cultists I saw. Keep in mind that killing enemies will spawn more, so even if they aren't Elite you may want to consider killing them for a chance at an Elite spawning in.

Got it in the same place as Echo.
Completed the game coop, so just went back to Act 1 of the Order of the One Light campaign and it popped towards the end of the act the first time I replayed it.
I do recommend playing it on a higher level so you get all the elites to spawn. I played it on combat level 4.
