Managed to get this on Act 1 of The Mask of Kelvin campaign, which was also the first time I encountered wraiths at all I believe.

Got it in the same place as Kez.
Completed the game coop, so just went back to Act 1 of the Mask of Kelvin campaign and it popped towards the end of the act the first time I replayed it.
I do recommend playing it on a higher level so all the elites spawn, I played it on combat level 5. But I think combat level 4 is sufficient.
