After obtaining the sword and shield from the Kiryat Priest as part of the story, and immediately after obtaining Thor's Hammer (100G) and getting the bitters, you'll be able to give the bitters to Warlock, the bartender, to have him make Pentatonic Gin. Take this downstairs to Sergio (guy on the far left) and give it to him, then mash 
through the dialogue. Head outside the Bar to the guy leaning against the building. Tell him the password - "going up and going down is the same thing" - to be let into a secret bar. Once inside, talk to the guy on the left end of the bar. You'll be eventually given the option to challenge another guy to a duel. With the sword and shield you will automatically win and unlock this achievement.
