If you keep a look out in chapter 6 out of the temple, you find two bars one of steel and the other of nagi steel( which if you haven't found yet, it's a katana like blade) keep going in till you hit the forge which is in a semi hidden tunnel.
1.drop your bar in the pot
2.rotate the pot using the wheel onto the embers
3.use bellows to heat embers
4.wheel melted metal unto sword mold
5.hit switch on the wall to release water onto the sword mold
6.retreive sword from mold, and place in ember till red hot
7.place sword on anvil and hammer
8.take sword to table and use leather strap machine
9.take your brand new sword

You can actually get this just by forging any 3 swords the game doesn't recognize the name just that you forged 3

I don't think you actually have to forge 3 different swords. I made 2 long swords and one sword of rage and the achievement popped up ;) (in chapter 5)
