I got this achievement as an assassin. I found 5 freeze scrolls by Chapter 8 right before you go into the prayer room.
You can also use a single scroll to slip multiple enemies and multiple times per scroll. I got the achievement by luring 2 necromancers together with my bow, casting the spell via scroll and appearing so that they would both slip on it. I manged to get the achievement in 2 scrolls but it was probably just a fluke :p.

I was also an assassin. In Ch. 9, I cast freeze once on some stairs with a narrow entrance as some undead were shambling at me. First two slipped and I got the achievement. So it might be a bit glitched. Or you don't have to be the one who cast the spell. I noticed other NPC mages had cast it earlier. Maybe all it takes is for it to happen in the game. Like when NPCs kill enemies, you still get the XP.

The easiest way to get this achievement is playing as a Mage. When you unlock level 2, you'll be able to use a 'Freeze" spell. Just 'freeze' the ground/floor and watch your enemies dying after little slide on ice.
You can also unlock this achievement using scrolls. But I am sure that it may be difficult to find 5 during playthrough. Anyway I failed although as a rogue I opened almost all chests.

I did this as swordsman, and only using two Freeze scrolls. I used one where the kind of desert village is at the outside entrance to the Temple where there are a bunch of orcs. Got 4 orcs to slip and fall on the same scroll. Used another scroll inside the temple on the numerous goblins inside and voila'.
