Additional tips:
- as mentioned in comments, make backup saves often!
- being a rogue increases a chance to collect all items in one playthrough (some of them are locked in chests);
- lot of items can be found in many places. As a warrior or mage you just have to spend more time to gather all collection.

What @FayMoon linked is the best (and only) guide around, fair enough, but oh man is it infuriating to try to figure out a slew of "go left go right" directions in a game like this. In addition, after the halfway point either the author was backtracking A LOT (I guess?) and thus often wrote left/right in reverse. Kept me searching hi and low for a few of these.
As luck would have it at the end, in Chapter 8, I got fuked hard by the Shadowsteel Armor chest key which I guess didn't register because I couldn't open the chest. Sure enough I had saved AFTER picking up the key... and fresh save slots had ran out in the previous chapter

Álso SAVE before you create a sword (ig sword of rage) because if it doesn't register then load it up and make it again (got that sword in ch 7 from the ingot near backpack.

The main reason I'm posting this solution is because when weapons don't register, people seem to panic and reload numerous saves to try and get them to register. There are multiple copies of most weapons through out the game.
Edit:There are in fact 'unique' weapons which can only be encountered once - just pay close attention when collecting named weapons. Thanks to Jack Knows for this information.
One of the glitched weapons in theShort Sword in Chapter 1. When picked up, for some people (myself included) it doesn't register in "Collection". You can access the collection via the 'start' button menu.
In Chapter 2 there are several Short Swords lying about - make sure that once you've picked one up, you check your collection to ensure it has registered. The first one I picked up registered fine for me.

The best advice I can give for this achievement is to make a habit of checking your collection after each small area to ensure all weapons are being registered in your collection. If not, keep a list and look out for them in following areas.

Also, be wary of picking weapons up in towns. Some are considered 'stealing' and the guards end up pissed at you. I found this out the hard way by picking up the Long Bow in the tavern in Ch1 and had to reload an earlier save because I had no idea they'd get mad, haha.

Thanks for the extra info, I'm still only on Ch.2 so I hope the unique weapons don't glitch on me.
