Once you get your glaive in chapter 2, simply carry on using it exclusively. Do NOT use any guns. The achievement should unlock when you reach the next checkpoint.

You can get this very easily at the end of Chapter Nine, during the boss fight with The Nemesis.

A "level" is each section separated by checkpoints, not an entire chapter of the game. Once you get your glaive, simply don't use your guns until the achievement unlocks.
