This is an unmissable story related achievement. You will acquire the Ashen Mist Heart by talking to or killing the Ancient Dragon at the end of the Dragon Shrine.

This was bugged for me as well. In the end, I reverted to an older copy of my saved data that was sitting on a different Xbox. I had to rush through a few previously-completed areas and speak to the Ancient Dragon again. That time, it did unlock. This achievement is extremely problematic.

This is a progress related, unmissable achievement gained either by talking to, or mercilessly slaughtering the Ancient Dragon at the Dragon Shrine.

Story progression. Can’t be missed. Once you have the King’s Ring head to the Ruined Fork Road bonfire in the Shaded Woods and then through the middle opening to access a gate (need the ring equipped to open). This leads to Aldia’s Keep, go through here to the Dragon Aerie and then the Dragon Shrine. At the end of the shrine you will speak to the Ancient Dragon and be given this item and the achievement will unlock. You do NOT have to fight him, but if you do so it will make the Vendrick fight much easier - only attack him AFTER you get the Heart.

Story related cannot be missed

For this trophy you will have to acquire the Ashen Mist Heart. This is given to you by the Ancient Dragon atop the Dragon Shrine. You need the King's ring to open the door down the middle path in the Shaded Woods, then make your way through Aldia's Keep, Dragon Aerie all the way to the Dragon Shrine and speak with the Ancient Dragon for the heart and the trophy.

There are no bosses in the Dragon Aerie or Dragon Shrine, so you won't have any trouble getting the heart.

At the end of the Dragon Temple location. To get it, you don’t need to kill small guards, only giants, otherwise the dragon will be your enemy. So just talk to him and he will give it to you. (The passage to memories where there are souls of giants will be unlocked)

Not true! Extinguish everyone! But if the golden dragon knights are not touched, then they will not attack.

If you still kill the dragon in the Temple of Dragons location, then in addition to the heart of the ash mist you will also be given the soul of a dragon, which is the fifth soul (along with the souls of giants), cutting off the defense and amount of HP of King Vendrick.
To easily kill a dragon, stand between the toes of its left hind paw (on the right side when looking at it) and move away when it stomps. All dragons and wyverns in the game are vulnerable to lightning damage, I advise you to enchant the lightning cannon from McDuff before going to these locations.

Story trophy. Simply talk to the Ancient Dragon at the end of the Temple of Dragons and he will give you the required item and a trophy along with it. Killing mobs in a location does not affect its aggressiveness.
